Why A Privacy Policy?

Lantex Insurance & Benefits Agency respects your individual privacy. This Privacy Policy
('Policy') embodies our commitment to its protection through adherence to fair electronic
information practices. This Policy puts you, the individual, in control of how your personal
information is processed. You have our promise that we will not electronically process
your personal information in any way that is incompatible with this Policy.

Information This Site Gathers About You

Lantex places great importance on visitor privacy and security. Lantex collects personally
identifiable information about you only when you provide it to us voluntarily, such as by
registering with us as a broker, completing a survey, or requesting information on the
Lantex web site. We do not require site registration by visitors and we do not pass
personally identifiable information about individual users to any third party without your
prior knowledge and consent. The specific types of personally identifiable information
and its use by Lantex are explained in further detail below.

Registration Material

Although registration is not required at the present time to use our site, at a future time
registration will allow greater access throughout the web site. We anticipate a broker
registration process to access insurance product quoting functions and at such time we
anticipate our site's registration form will ask users to give us basic contact information
such as name, company, address, phone, and e-mail. Brokers will be asked to provide
state license numbers and provide Lantex a copy of their license before full access is
granted. We may use the contact information to contact the visitor when necessary.
Users may opt-out of receiving future mailings (see the choice/opt-out section below).
When broker registration is activated, we anticipate a login process will provide unique
identifiers to verify the user's identity along with various demographic and profile data.

After you have signed up for our services, we may be in communication with you about
your account. Those communications are essential to our relationship with you and to
our ability to provide you with quality services that are responsive to your needs. For
operational and quality assurance purposes, we take note of whether and how you use
the information and services that we provide, such as by recording site traffic patterns
and by maintaining log files of users' access to site files.

Hyperlink User Pre-Registration

When a broker activates a hyperlink with one of Lantex's affiliated MGAs from the Lantex
web site, that broker's registration User ID may be shared with the MGA's web site in
order to accomplish pre-populated registration on behalf of the broker, thus saving
re-registration by the broker at the linked site. No other user information provided in the
Lantex web site is shared.

E-Mail Addresses

Lantex receives e-mail messages from our users. We do not sell or otherwise release
the names or e-mail addresses of users who write to the site. Your correspondence with
us remains private.

Advertising

We do not use third-party advertising companies to serve ads on our web site and other
web sites.

Cookies

A cookie is a piece of data stored on the user's computer tied to information about the
user. Usage of a cookie is in no way linked to any personally identifiable information while
on our site. At the present time, cookies are not used by our site. However, we anticipate
using both session ID cookies and persistent cookies. For the session ID cookie, once
users close the browser, the cookie simply terminates. A persistent cookie is a small text
file stored on the user's hard drive for an extended period of time. Persistent cookies can
be removed by following Internet browser help file directions.

We anticipate setting a cookie on our site so that users would not have to log in a password
more than once, thereby saving time while on our site. If users reject the cookie, they may
still use our site.

Log Files

Like most standard web site servers we use log files. This includes internet protocol (IP)
addresses, browser type, internet service provider (ISP), referring/exit pages, platform type,
date/time stamp, and number of clicks to analyze trends, administer the site, track user's
movement in the aggregate, and gather broad demographic information for aggregate use.
Log file information is not linked to personally identifiable information.

Sharing

We do not sell or rent personally identifiable information.

Legal Disclaimer

Though we make every effort to preserve user privacy, we may need to disclose personal
information when required by law wherein we have a good-faith belief that such action is
necessary to comply with a current judicial proceeding, a court order or legal process served
on our web site.

Aggregate Information (non-personally identifiable)

We share aggregated demographic information with our affiliated MGAs and business
partners in order to provide specific services. This is not linked to any personally identifiable
information.

Service Providers

We partner with other affiliated MGAs and insurance carriers to provide specific services
to brokers. For example, they provide insurance product quotes, application processing
and application forms. When the user signs up for these particular services, we share
names and other contact information that is necessary for the third party to provide these
services. These third parties are not allowed to use personally identifiable information
except for the purpose of providing these services.

Other Third Parties.

We do not share personally identifiable information with other third parties.

Business Transitions

In the event Lantex goes through a business transition, such as a merger, being acquired
by another company, or selling a portion of its assets, users' personal information will, in
most instances, be part of the assets transferred. Users will be notified via a prominent
notice on our Web site for 30 days prior to a change of ownership or control of their
personal information. If as a result of the business transition, the users' personally
identifiable information will be used in a manner different from that stated at the time of
collection they will be given choice consistent with our notification of changes section.

Security

Our users are given the opportunity to 'opt-out' of having their information used for purposes
not directly related to our site. Users who no longer wish to receive our newsletter and
promotional communications may opt-out of receiving these communications by sending

Third-Party Personally Identifiable Information

Lantex may request that brokers or individuals submit census data regarding prospective
policy insureds in order to use the rate quoting functions of the Lantex website and/or that
of its affiliated MGAs. Census data generally consists of the prospective insured's name,
gender, birth date, zip code of residence and workplace, and number of children. In addition,
we may request that brokers provide certain information about the medical condition of
prospective insureds that will be used in generating the requested rate quotations. This
information is used solely for the purpose of generating rate quotations at the request of
the broker. The information is not shared with any third party, except when requests are
sent by the user for rate quotes. In that case, the information is shared with the affiliated
MGA and insurance carriers working with Lantex for the purpose of providing the requested
quote.

Links To Other Sites

Our web site may contain links to other sites. Please be aware that Lantex is not responsible
for the privacy practices of other web sites. We encourage our users to be aware when
they leave our site and to read the privacy statements of other web sites that collect
personally identifiable information. This privacy statement applies solely to information
collected by the Lantex web site.

Security

Only those Lantex employees that have a legitimate business purpose for accessing and
handling personal information obtained by us are given authorization to do so. The
unauthorized access or use of such information by a Lantex employee is prohibited and
constitutes grounds for disciplinary action.

Additionally, our information management systems are configured in such a way as to block
or inhibit employees from accessing information that they have no authority to access.
Non-employees do not have access to personal information from our web site.
